Case managers at HARC provide information, consultations and referrals to the community resources that will best help people living with HIV/ AIDS.  Case management services are immediately available upon completion of an intake interview.  Case managers assist with the development, implementation, and evaluation of health and support service plans; and are also responsible for locating, coordinating, and monitoring cost-effective, timely delivery of quality services.  Case managers serve as a central point of access for the needs of people living with HIV/AIDS in HARC’s service area.

All case management services are provided at no cost by case managers who have been certified by the Michigan Department of Community Health.  If you or someone you know has recently been diagnosed that they are living with HIV or need assistance, please call HARC and ask to speak to the case manager on call.  We are here to help.
